This includes insight in new parameters from real-world data for monitoring COPD and CCCs, AI-prediction models for flare-ups of COPD and CCCs, a privacy-preserving platform, and self-management strategies. The project will present its open science strategy for follow up. In addition, the team will provide guidelines on how personalised digital health tools can be integrated in pan-European chronic healthcare settings.
