On 31 August 2025, the RE-SAMPLE project has officialy concluded.
RE-SAMPLE started in 2021 under the leadership of Professor Monique Tabak (UT). Amid the challenges brought by the COVID-19 pandemic, the consortium's resilience and commitment ensured that the project has not only realised its ambitions, but has also created outcomes that will continue to inspire and resonate well beyond its lifetime.
Over the 4.5 years, close collaboration with patients and healthcare professionals was at the heart of project work. Their involvement was crucial in shaping self-management strategies and co-developing the Virtual Companionship Programme. At the same time, respect for privacy was a core principle in building the RE-SAMPLE platform.
The project leaves a significant legacy, including new parameters from real-world data for monitoring COPD and CCCs, AI-prediction models for exacerbations, a privacy-preserving platform, self-management strategies, and guidelines on how personalised digital health tools can be integrated into pan-European chronic healthcare settings.
To make these achievements widely accessible and to inform future research and practice, the consortium has produced a range of publications, video resources, and open-access materials.
If you would like to learn more about the outcomes of the RE-SAMPLE project, you are cordially invited to join the project's closing webinar hosted by the EU Health Policy Platform on 17 November 2025, 14.30-16.30 CET.
